Las miopatías inflamatorias idiopáticas en los adultos hacen referencia a cuatro condiciones patológicas: polimiositis, dermatomiositis, miopatía por cuerpos de inclusión y miositis autoinmune necrotizante. La polimiositis y dermatomiositis son dos enfermedades sistémicas con una gran variedad de síntomas, que se caracterizan por la presencia de debilidad muscular proximal y evidencia de inflamación muscular. Una condición nosológica adicional denominada dermatomiositis amiopática se refiere a pacientes que tienen características cutáneas de dermatomiositis sin debilidad muscular ni alteración en las enzimas musculares. La característica clínica cardinal de las miopatías inflamatorias es la instauración progresiva (semanas a meses) de un cuadro de debilidad muscular más proximal que distal, presente en el 90% de los casos para polimiositis. Usualmente hay compromiso inicial de la cintura pélvica generando dificultad para subir escaleras, o para levantarse de una silla sin apoyabrazos o del inodoro. El compromiso de la cintura escapular genera dificultad para levantar los brazos por encima de la cabeza, dificultando tareas de la vida diaria como peinarse el pelo. A nivel de la columna cervical la debilidad muscular puede generar dificultad para levantar la cabeza de la almohada. El paciente puede referir además la aparición del exantema cutáneo relacionado con la dermatomiositis, el cual se puede presentar en forma simultánea o después de la aparición del cuadro de debilidad muscular, salvo para el caso de la dermatomiositis amiopática, en la cual, como se describió previamente, las manifestaciones cutáneas aparecen sin compromiso muscular.
MioApp es una aplicación que incluye contenidos teóricos y calculadoras de diagnóstico y clinimetría de interés para el médico con respecto al tema de miopatías. La aplicación está dirigida a dirigida a médicos generales, especialistas en medicina interna, medicina familiar, fisiatras, neurólogos y reumatólogos.
La aplicación también incluye contenidos sobre enfermedad de Pompe, con el objetivo de recordar al clínico la importancia de incluir este diagnóstico diferencial dentro del abanico de posibilidades en los pacientes con cuadros de miopatías en estudio.
