Créé par l'association Handidactique, il évalue les améliorations permises par la charte Romain Jacob dans le système de soins. Cette charte a été signée par toutes les régions de France, et rassemble 12 points d'action pour améliorer la prise en charges des personnes vivant avec un handicap dans le système de soins.
Le questionnaire est composé d'une vingtaine de questions simples et rédigées en Facile à Lire et à Comprendre (FALC), est destiné à être rempli par le patient à l’issue de chaque rendez-vous avec un professionnel du soin.
Ces données, complètement anonymes, permettent après analyse de dégager les problèmes subsistant au sein du parcours de soin et aident l'association Handidactique à adapter ses actions à des problèmes plus spécifiques.
Les résultats sont publiés tous les deux mois, et consultables via l'application ou le site de l'enquête Handifaction.

Created by the Handidactic Association, it evaluates the improvements allowed by the Roman charter Jacob in the system of care. This charter has been signed by all regions of France, and brings together 12 action points to improve care for people with disabilities in the healthcare system.
The questionnaire consists of twenty simple questions written in Easy to Read and Understand (FALC), is intended to be completed by the patient at the end of each appointment with a care professional.
These data, completely anonymous, allow after analysis to identify the problems remaining within the care path and help the Handidactic association to adapt its actions to more specific problems.
The results are published every two months, and can be consulted via the application or the website of the Handifaction survey.
