We power primary prevention by delivering guidance and inspiration on the patient’s journey to live a healthier life.
This app is only available through your Primary Care Provider.
The app will provide you with real-time interpretation of the information and small tasks you can do to improve your measures while giving you a place to track your improvements. This will all be connected to our PreventScripts database which will help you and your provider work towards preventing diabetes, hypertension and obesity.
