About us:
Life Circle model is centred around a trained nurse supported by family doctor and aided by systems and processes required for rigorous and regular monitoring and follow up of an enlisted clientèle of senior citizens and bed ridden. The core of the model is a comprehensive health assessment at home for each senior citizen. This is to be done by the nurse supported and guided by the family doctor. The nurse is to work with the senior citizen, the caregiver, and the family doctor to customise an evidence-based comprehensive health plan.
